Please fill out the required information. After you hit submit, you can review your answers and will also see a file upload button. Please upload your ethical standards disclosure forms (all co-authors as well as the first author must complete the form). You may also upload the abstract as a word or PDF file and any other supporting documents.

Please note: Abstract must be related to the field of fertility.

Please note: No abstract submissions can contain clinic names.

Abstract style:

Type the title in bold, authors in italics, and body in normal text. Abstracts should be no more than one page. 

Do not use all capital letters and do not indent.

Use a clear, legible 12 pt. black font (e.g. Calibri, Arial, Times New Roman).

The title should reflect the contents and should be no longer than 150 characters.

List all authors first name followed by family name followed by affiliation.
(i.e. 1 John Campbell, 2 Margaret Witherspoon;
1 University of Toronto, Ontario, Canada; 2 Fertility Clinic, Red Deer, Alberta, Canada)
